Another difference: in the second file there are several blocks with span="all", while in the first file there is just one of them.

Removing span="all" (which anyway seems unnecessary, as the body just has one column) solves the problem.

A couple of notes about the fo file:

- there are a few space-before and space-after defined using just the .optimum component; I was wondering if this is correct (isn't the default value for .maximum and .optimum 0 points?)

- there is always an empty block at the end of the foontote-body; at the moment it does not have any effect on the output, so they could maybe be avoided

I played around with some examples I found in the FOP-distribution and somewhere else online, I think I have to clean up the code when it's ready. Already bought a XSL-FO-book today ;-)
