Please note that FOP 0.20.5 and FOP 0.94 are two very different beasts. There are separate Barcode4J plug-ins for both versions. If you include barcode4j-fop-ext-complete.jar in the classpath instead, it will work. The file you used is for old FOP 0.20.5 as the filename says.
