Could have been a regression introduced by my refactoring the handling
of block-containers between 0.94 and 0.95beta. The case wasn't covered
in our test suite. I've committed a fix to the 0.95 branch:

The problem was that the flow area didn't generate a real reference area
(i.e. a new coordinate system).

Please note that the width="100%" will still (i.e. like in 0.94) return
a value that may not exactly be what you need for such a column
separator. I don't think there's any construct in XSL-FO that allows you
to express that properly. FO-Implementations like AntennaHouse have
proprietary extension for that.
