Ivan,
You would be correct. The annotation in schematic has nothing to do with
position. It is indeed related to order of placement. This practice is
consistent in 99SE as is was in Client 3.x. My question is why would you
care what order the annotation is in schematic anyway? The back annotate
feature works from re-annotating the pcb then back annotating the schematic,
not the other way around.

I always thought the annotation was in the temporal order in which parts
were placed in a schematic.

The first U? you place will be U1, the next will be U2, etc.

It doesn't seem to have anything to do with the X,Y coordinates or sheet #.

Of course, that is for Protel SCH 3.X.  I haven't made any new schematics
with 99SE yet to see how it works.  Probably the same.
