When I create a folder and file with special characters in the name on
my Ubuntu machine, for example:

MañósosCarácteres
|-cönáñón

And then add another file to this folder on my Mac, for example:

MañósosCarácteres
|-cönáñón
|-éstedesdemác.rst

A second folder with exactly the same folder name "MañósosCarácteres"
will be created on my Ubuntu machine the next time I update, only one of
them containing both file. From thereonforward it just gets messier and
messier, sometimes new names with the accents going the other way will
be created on my mac. All in all versioning breaks down.

I saw a UTF-8 setting for the database in the configuration settings
(through the UI) but this is already turned on.

Is this a configuration issue? Or can fossil not handle special
characters in file and folder names?

The wiki pagenames do not handle special characters gracefully either. I
have not come accross any issues at all with the content of files or
wikipages. Only with the names.
