What does your numbering set up look like? Do you have more numbering sequences in your files? I've seen similar behavior when auto numbered text is in anchored text frames, in those cases numbering sequence is not linked anymore. Also, be sure there are no manual numbering adaptions (can occur when someone tries to correct a numbering problem by setting the number manually, but forgets about it when chapters are reordered) FrameMaker never randomly changes numbering. There's always an error in file set up if these things occur.
