This is old-school, but.... If your line spacing is standard, one option would
be to place a new flow on the master page(s) just wide enough for the largest
line number.
On the body page(s), you would insert into that flow a series of autonumbered
paragraphs with no other content. (They should have a different flow ID than any
other series of numbered paras in the document.)
If you needed to adjust for spacing around heads, you could create a set of
"head number" paras that would use the same flow ID, but are tweaked to match
the head spacing. The only problem with this is that applying the para tags
would be a manual process. You'd have to do a similar thing for figures.
