The page includes one frame for the main chapter head only and one
frame--for the rest of the page--that includes paragraphs and
subsequent heads, including level 1 heads.
I suspect that an errant end-of-flow tag in the chapter head frame
may be the problem. No idea how it got there, it appears in only a
couple of my chapters. The resulting TOC behavior is inconsistent,
but I will test to see if this, or something else, may be the culprit.

Hi Karen, if you say : 'In the document, the frame in which the
misplaced level 1 head resides is
not positioned above that of the chapter head.' do you mean that
you're using separate frames, just for headings or for the contents
as well?
We once had an issue with frames and numbering and it turned out
that not the order of frames was important, but the sequence of how
the content was added.
What happens if you switch chapters, does always the last chapter
give the same problem?
