I start with a blank document. I create the format, then save it in a folder reserve for formats. I name the file based on the format name, and I append a (p) or (c) to the filename to remind me whether it's a paragraph format or a character format.
Later, when I need to import the format into a document, I just open the template file and use the File > Import > Formats function.
