Log message:
Okay, Lots of little bug fixes here.  Suffice it to say that you won't
get any meaningful midi output without 'em.  Also incldes a heavy dosage
of sanity checking, and better debugging output.

Now I want you folks to bang on this!

Give me some feedback now that I know it all works.

...and a tweak to .cvsignore to ignore config.h (autogenerated)

What doesn't work:  HQ/QFG1/Iceman (the "v-1" patch.001 format)
ALSA (hasn't been moved over, or put into autoconf)
