Log message:
Touched a lot of files, but relatively minor changes along two axes:

1) midi generation/translation code now understands the concept of 'play
flags' of the device.
2) soundserver now tracks the polyphony of each channel.

Next up:  Abstract out the midi_mt32_* calls into a more general
'midimap' API.
Figure out what's up with Iceman/QFG1/Camelot.
