Whee, the fun continues!

1) Added pitch bend to the 'mt32gm' midi device.
2) Now you can specify midi_device and midiout_driver in the config file
and on the command-line.

midi_device = mt32 | mtgm [ | pc-beeper | adlib | etc.. ]
midiout_driver = null | unixraw | alsaraw [ | serial | etc.. ]

Or, the args -Onull -Mmt32 will also work.   See README and sciv -h for
all the details.

(hmm.  we need to update the man pages with all of the new options..)

Anyway!  Let me know how the mt32gm device sounds.  I know it has a long
way to go before it's worth boasting about..
