Significant update to the fm emulator; basically tracking the code
changes in adplug.
It shouldn't be any more broken than it was before; 12-voice operation
still works.. (I'll get around to 16-voice emulation one eventually..)
Meanwhile, I think some of the volume levels are more sane now, thanks
to the core emulator changes.. SQ3 sounds better, but still not
quite right. Percussion in particular still seems louder..
