Came across a glitch in the 12-voice code in the fm emulator -- we were
actually dropping the extra three voices, and thanks to a few hard-wired
voice counts, adding in the extra three to the mixer caused a segfault.
Meanwhile, I can 't find the reason for the pcmout_sdl crash..
BTW, there seems to be a glitch with the CVS server; my connections hang
at the end; it's a bug in cvs, I'm told...
