I'll try to be more specific:

Someone shows up who wants to join your medical insurance program. What
things are they asked about that will determine whether they are admitted to
the plan or not?  I understand that the plan seeks members who have a
health-conscious lifestyle; I am interested in what specific criteria the
plan uses to determine if the individual does indeed have such a lifestyle.

Basically, the idea is to have a health-conscious lifestyle.  This can be
assessed on a heuristical basis.  It is fostered by the insurance's member
magazine with good insider advice, which other "health" insurances don't
bother to provide to their members (what a clever way of "saving" money!).
