I'll try to be more specific: Someone shows up who wants to join your medical insurance program. What things are they asked about that will determine whether they are admitted to the plan or not? I understand that the plan seeks members who have a health-conscious lifestyle; I am interested in what specific criteria the plan uses to determine if the individual does indeed have such a lifestyle.
