It's difficult to tell from afar why a window has been placed
where.  Please add this line to your config file:

  bugopts explainwindowplacement

And then open the offending window.  Fvwm will then print an
explanation of how the window was placed to the console.  If you
can't figure out what's wrong from that output, post it to the
mailing list.
