On my Pismo 400, I've never heard a cooling fan. On my iBook G4, I  
have heard it when the computer gets hot. As for testing the fan... On  
the iBook you can hold down the power button until you hear a long  
beep, and then the fan will blast. I tried this on the Pismo, and the  
fan didn't come on, however there was a beep. I assume even if the  
Pismo fan was not functioning right, it wouldn't be too big of an  
issue.
