I installed a 4 port USB card (cheap one, has a chip labelled 'Agere') into
a CW Boston mac clone. I had the same problem as you, that it would have an
error 11 on startup with the USB adapter card drivers installed. If I
removed the 'USB support' extension (I think it was) or the card, it would
boot okay. The card is labelled as being mac compatible, but that it needs a
G3 or better. Perhaps it requires PCI bus rev. 2.1 (only in beige G3 and
later).

So I will try the card in my 7600 and my beige G3. I have USB card drivers
1.5.6 which came with OS 9.2.2. What is the chip on your card?
