It was running OS X Jaguar on a six gig drive. I don't know what you mean by "journaled" and no, we didn't do any firmware updates.
Your suggestions sound like good ones. Thanks.

Was this system running OS X before you installed the new HD (and controller)?
Did you update the firmware to the latest version before installing OS X?
Is the drive journaled?
Is your PRAM battery good?
