Is it by any chance one of the original big fat Silver and Gray optical guys with the two side buttons? These ones are known to suffer wire failure just were it exits the housing at the nose of the mouse. My dad managed to fix mine and it works OK now, but it was a known issue. I dunno if MS did replacements, I doubt they'd do one now.
