The solution for me was to flash back to an earlier version of the firmware. I received ATTO's firmware v1.44 from their tech support and once I re-flashed to v1.44 the problem went away. I don't know if this old firmware works in OS X. I have been using the card since May 2003 with OS 9 with no problems. I use the rather expensive pink Amphenol 68 pin U2W/LVD twisted cable with an Ultra 160/LVD terminator plugged into the last connector at the end of the cable.
