I did insert it many times; even alternating it back
and forth with rev A & B and rev A consistly worked
whereas Rev B did not. Strange.

There's not much to go wrong with those ROM modules.

First, check that the contacts are clean. You might give them a cleaning with a swab and some 91% isopropyl alcohol (usually next to the 70% (rubbing) alcohol at the store). Do not leave any stray fibers behind.

Second, it is possible though unlikely that a pin has come loose. The chips are soldered to the board and occasionally, the board will flex enough to break or crack a solder joint or joints. If you have a magnifier handy, you could examine the solder, though this won't help you much unless you can also solder or know someone who can.

Third, the ROM chips may have been zapped by static electricity. If this has happened, there's nothing you can do short of replacing the chips on the ROM. If you have the 2-chip ROM (by far the most common) then there's no point. The 32 bit wide 72 pin chips are not available. If you have the much more rare 4-chip ROM, then there are a variety of Flash chips that can be substituted for the original ROM after they've been programmed appropriately.

Fourth, (very long shot) how much RAM do you have installed. I've found that machines with more than 384MB installed won't boot if pin 112 (that's from memory, so could be off) on the ROM is connected. Most ROMs are configured correctly, but there are a few rare ones out there.

With the Rev. B installed the machine does not even chime, correct?
