Yes, it's always lit up red.

I did bite the bullet, and reset the PMU. The reset made <no> difference,
so no internal speaker and thus no chime on startup.

The internal speaker is disabled when any speaker/headphones jack is plugged into the speaker/headphones port on the back. If this speaker/ headphones port is physically damaged it could possibly not enable the internal speaker to work properly. If this is the case, you might be able to "finagle" or bend some contact (WITH COMPUTER OFF) to get it working again if you can see what's wrong. Otherwise, the port would need replacement.

Have you tried booting with speakers or headphones to see if the chime is heard? Note, the volume must be turned up if the speakers or headphones have manual volume adjustment.

There are two places in OS X where sound is controlled, one is System Preferences>Sound, and the other is Applications>Utilities>Audio MIDI Setup. You might check both to be certain a "mute" box isn't checked?

It's possible the startup chime was disabled via Terminal command or by a 3rd party Preference Pane or app? There is a special System Preference Pane called "Startup Sound" which can custom control the volume of the startup chime, so you might want to install this and see if it has any effect:

I believe all sound is handled in AppleHDA.kext. Your optical port is normal, there's nothing for you to mess with in this kext.
