The fans will ramp up if the CPU is hot. My 1.25 Ghz Dual Processor MDD is as quiet any other Mac I have used as I write this post. The room temperature is 82 F (28 C). When the room temp is much higher you will hear the fans.I believe the fans in my MDD are stock. It is important to blow or vacuum out any dust bunnies as they will retard the cooling. When I received my used MDD a dust clomp the size of a mouse (the rodent) was lodged under the Darth Vader grill at the bottom of the case. There is definitely a fan noise on a boot or restart when the CPU is working hard.
