You're correct on the portable bit, they don't even like running in windows 
that require admin rights unless you actually run nvda.exe as admin.
Installed copies will run fine on the uac screens. When you select to run on 
the logon screen, you're also setting it up to run on secure screens 
(because that's what login is).

Not really - as I understand it is that access to contents of UAC popup is
literally being denied, due to nature of user-access-control - same way, a
while ago, some of the anti-virus software thought some screen readers were
acting like trojan horse malware, due to them scanning textual content of
other windows/dialogues.

And, don't really think logon screen activation has much to do with it -
this was after had been working in main window/desktop area, and, since run
portable version of NVDA, it's not necessarily too likely to cooperate with
logon screen startup...?
