Those of you having these problems, could you give us an idea about the
amount of mail traffic that is going through your proxy? How many
delivered messages/day, how many rejections/day, etc. These numbers
would help us as we're investigating these reports.
I typically see 5-6 thousand messages a day through my proxy plus 2-3
thousand rejections from the RBLs that I use (up to a few spikes of ~20-30
thousand rejections/day). I've not seen any RBL failure except a few
weeks ago when osirusofts servers went offline.

This is always be the case...not all spam sources are, or can be, listed
in the RBLs. Spam from yahoo/hotmail/msn/etc for example is very hard to
stop using IP blacklists since blacklisting those servers would result in
blacklisting ALL yahoo/hotmail/msn/etc email.

Is your internal email server doing RBL checks using the same RBLs you
have your firewall configured to check?
