Troy,

I think it depends on the individual gecko, whether they're juvenile or 
adult (I think juveniles are probably more prone to drop their tails more 
easily) and the level of stress involved.  I think this would happen at the 
moment of stress (it did for me) and is probably also more likely if there 
is some kind of pressure on the tail.  However, I think I've heard of this 
happening without contact with the tail.
