Here's one that might even sell as shareware.

I have a very slow dial-up connection to the web, and only
one phone line there.  So when I forget to hang up the connection,
our phone line isn't available for calls, faxes, etc.

Suppose the DUN icon on the desktop blinked red when connected.
I would be more likely to log off.  When some app is open, the icon
would be covered, so no problem.  So an icon that could be set to
change to blinking red when the status was connected, but the
usual when disconnected would be helpful.

Might even be OS independent.
