Roland, Boris, Good news for you, bad news for me. When I switched a different machine over to a Tavor HCA, the HCA came up as expected. So, the problem is either with the particular machine or particular HCA.
I'll keep playing to see if I can isolate the important factor, but it doesn't look like an OFED problem.
