At http://www.gentoo.org/doc/en/gentoo-upgrading.xml you will find the "default" explanations on how to upgrade from one profile to another; there you will also find a link to http://www.gentoo.org/proj/en/base/amd64/howtos/index.xml?part=1&chap=1#doc_chap1 which describes the upgrade process specifically for amd64. I followed it yesterday and have no problems at all, even after reboot.
