I don't understand the confusion.  There are 4 ebuild in question. 
Only 2 seem to be of particular importance in this scenario.  Those
are squid-2.5.7-r5 and squid-2.5.6-r3 (at least that's what it says in
my recently sync'd portage tree).  Now, when you put
>www-proxy/squid-2.5.7 it wants to go down to the 2.5.6 version
because the other option (2.5.7-r5) is higher.  Same with the other
case.

What version do you wan to be installed?  What are you attempting to mask?

Perhaps you have 2.5.7-r2 because of an older ebuild.  In that case,
if you put >www-proxy/squid-2.5.7-r2 it shouldn't downgrade (because
you aren't masking the currently installed package).  If you have the
equals there as well, then you are masking your current install and
it's going to want to force it down.
