Suppose I want to make a large number of digitised topo sheets available

Now, suppose there are occasional (but important) administrative changes to map boundaries and other features. The changes are likely to be very
localised. Can anyone suggest a good way to handle that?

How about TileCache - http://tilecache.org/

You can create a cache of your topo maps and then only change the tiles as needed.

See also the great GeoServer tutorial on TileCache at
http://docs.codehaus.org/display/GEOSDOC/TileCache+Tutorial

It talks about using expiration times to control the clients' caching.
