One thing I found when it came to shopping online for my fathers
progressives is that many of the online places require a larger frame
and lens size so the segments aren't very narrow and they can fit a
wide variety of customers with little hassle.  In the end, most of the
issues with my father came down to the actual segment size which had
less to do with PD and more to do with the actual brand of the lens
and his ability to get used to them.  Don't get me wrong, PD is very
important, but brands like Younger and Varilux seem to be less
troublesome for him.  For his use, he found ClearLight Lenses to be
perfect and he felt better when 39dollarglasses told him that he could
have them re-segmented.  Beyond the actual frame size, no real fitting
was necessary and just his PD worked perfectly.

With that said, one thing that helped is that 39dollarglasses sent him
frames to try on and whatever he didn't like he sent back.  They were
really accommodating even going so far as to let him try many of them
at no cost.  The ones he liked, they put lenses in for him and he was
thrilled, saving over $300.  In all, I think they might be able to do
what you're looking for so I would give them a try.  Anyway, I hope
