It's easy. Most of the online forms are already arranged so that the fields
line up with what you wrote.
The columns, in order, are SPH (sphere), CYL (cylinder), AXIS, and ADD
(bifocal)
Your right eye has a prescription of SPH -2.75, CYL -1.00, AXIS 45
Your left eye has a prescription of SPH -1.00, CYL 0.00, AXIS 00
ADD +1.50
The zeroes are sometimes written as PL, which means plano.
