I'd draw the line based on probability of damage to others.
Airline pilots
Mandatory retirement at 67.5, First class physicals every six months
till age 60 then subtract a month from the interval for every year over
sixty. Add a cognative test at each physical. Perhaps a cognitive test
before each flight.
ATP not flying passenger airlines - ditto limits above but no mandatory
retirement.
Commercial pilots
Second class physicals every year.
Private pilots allowed to fly heavy metal
Third class physicals every 3rd year till age ??? then every 2nd year.
Sport pilots and Recreational pilots - limit one passenger who must be
informed of lack of medical certificate from the FAA - Self certify
medical condition and be required to have a valid drivers license to weed
out stupid drunken drivers and those chronically unable to abide by laws.
Ultralight pilots - no passengers - go have fun and stay away from
congested areas and at least 500 feet from any person, structure or
vehicle.

Wherever you draw the line, it's going to be controversial. So I say draw
it
a bit on the early side. Heck, I'd go for 50. Or even 40 (I'm 43). Or
don't
draw it at all and have people drop in on the AME every 2-3 years.
