Isn't this the general situation, namely that it doesn't fit
on the page. In that case, you would have to deal with the
page layout anyway, e.g. to avoid that the music collides
with the page numbers and the copyright marks at the bottom
of the page. Introducing hacks to force the title and the
first staff is just a workaround to the symptom, it doesn't
solve the problem.
All your suggested solutions have problems, since we still
want to be able to run the Lilypond output straight through
TeX and get the music correct (although without titles).
I'd prefer to add a note on this in the FAQ, unless you
can point to some situation where this happens and it's
not caused by the page layout.
