Have you tried adding spaces ? eg. "Flute          " (it's a kludge,
but it might work)

Yes, this is standard typesetting. If you want to change this, change
the definition of  break-barline  in scm/lily.scm

Yes, you should have been putting the two together in the same Voice
(not staff).

No, but there are other numerous parameters to tune spacing. Have a
look at ly/params.ly
