Editing titledefs.tex should work, are you sure that you edited
the file used by LaTeX and not some other copy in the directory
tree (see the printouts from LaTeX)?

Setting the property dynamicPadding used to work, but as far as I
can see, it doesn't work in the latest versions. Bug?

Yes, the dynamics were included in the MIDI output in version
1.3.30 (see the change log). The settings are done in midi.scm.
