I emailed David off list originally, but I'll mention a little of it here
since there seem to be others running into similar issues.

Out of 55,000 accounts, creating a couple hundred new ones a week we've run
into the same basic issue from time to time.  You try to create an account,
get an error indicating it couldn't be created because it already exists,
but when you look it up in the control panel it doesn't show up there and if
you do a retrieveallusers it doesn't exist there either.

Short answer, we've opened a support case with google on each of these
issues.  In the 3 or so we've run into they have been in some sort of
limbo.  The account is sorta there, but sorta not.  Google techs have had to
delete it from their end, after that, the next time our provisioning piece
runs it creates them fine.
