They should all still be there within the document level (you could name the docouments in the kml better perhaps? and you can also over-ride the icons with your own) It might also be fairly simple mod to give it a setting to not show document level icons... (its already skipping file level legend entries)
by the way. this is a css I use to make things cleaner looking... http://www.dyasdesigns.com/geoxml/gmap.css The significant error for me is the ground overlays bit I willl see about fixing that this eve.
