It's a rolling 24 hours; the rate is measured over the immediately preceding 24 hours. If you do trigger a lockout, it lasts for 24 hours from when you trigger it. At least, that's what reported experience seems to be. X-FORWARDED-FOR is useful where you are connecting through a proxy, as geocoding limits are based on the client IP address which is presented to Google.
