looking at your form it still looks like geocoding.
You still don't seem to have a  map .
As the new page is still php we have no idea what is happening.
>From Google position you seem to be breaking 2 of the TOS ie not
displaying map and over the limit of geocoding.
Until you resolve both issues satifactory I cannot imagine the service
being restored
