On second thought... I see where the requirements come from.

 Jim points out that we can precoordinate that into the domain ontology or we 
can consider
 whether we want to capture the semantics in the i2b2 model

Anybody have a preference on how to represent stuff like
 - maternal gestational diabetes
 - maternal BMI
?
