We have run the EMR query and have the demographic results ready. Slide
16, 2. says "add emr_data form to your working copy".
Does that mean we add just the EMR data fields, which I think are rows
82-94 in this data dictionary csv:

to the ALS Clinic Survey form? Or do we import the entire data dictionary
csv?
