**Enhancements**:
1. added parameters `start_date` and `end_date` so that inclusion time
window can be customized.
2. added parameters `driver_type` for oracle sites who may have different
preferences on db drivers between **OCI** and **JDBC**. Note that **JDBC**
is currently the only option for sql server sites.
**Defects**:
1. sites may run into "Java heap space" error, which is an out-of-memory
error whenever the JVM reaches the heap size limit. If that occurs, site
can decide to restrict collection window by resetting `start_date` and
`end_date` (e.g. start_date = "2014-01-01")
