The education value is vital - people need realise the extent of health expenditure. I always tell them how much their pacemaker costs etc so they appreciate what the guvmnt is doing.

I do bulk bill some people, but generally all pay a small gap over the fee ($10 minimum) and they can then go to medicare and haggle for their refund.

A prominent knee surgeon once wrote to the SMH and said that he set a fee that he felt reflected his efforts. The patient then had the right to claim some back from their insurer (Medicare). IF they felt their rebate was insufficient then they should discuss it with their insurer, not him.
