I would start with an "address book" as core module - a database of 
demographic data, and modules to browse, access, add and edit information

Then a simple document repository (images (scanned / photos), html files, pdf 
files etc.) - simple to implement, and bloody useful because I know of not a 
single commercial application that has done this part well

Then an appointment system - again well defined, simple enough to implement in 
isolation

and so forth
