What about a Wacom tablet that produces a tiff or whatever in Paint, Gimp, Photoshop, etc. that is then imported into a Letter Writer template in MD, with appropriate patient demographics field placeholders.

After all the fields for the patient demographics will have originally come from PS, if that's what you use. If he wants to export it into another database system, export is supported, but I guess that depends on the system.

If he wants to use it on his desktop, it's quite compact, depending on the size Wacom he wants.
