But no funding or Division involvement is required. 
Here's my doctor's bag for out-of-surgery visits:

.. Laptop, mobile printer (Canon bjc-55), wired and wireless
connectivity to any Internet connection, and onboard modem
and dial-up account using 0198 phone access number (for
price of local call anywhere in Oz) - so all that is needed when
out of the surgery (such as at nursing home) is an Intermet
connection or phone plug.  At my surgery is an ADSL Internet
connection to the Winconnect XP server (XP pro + Winconnect
which provides terminal services) which is cheaper than Windows
2003 server with terminal server licences. 

All practices should have something similar.
