I have found the Communicare system to be quite a good robust program.
It was originally designed for the NHS and has a chronic disease/management/reporting  focus. It is the only program that I am aware of that has an interface for networking records over the net (HealthConnect). It is well adapted for a team environment and uses an OSS database (I think)
They dont seem to have marketed to mainstream GP's though.
